Life and AD&D Insurance
Having peace of mind knowing that you or your loved ones are provided for in the event of serious injury or death is important. We provide Life and Accidental Death & Dismemberment Insurance through New York Life.
Our Life and AD&D insurance is now provided through New York Life (formerly Cigna). Though the name of the carrier is changing, your benefits and costs are not changing.
How does it work?
Life and Accidental Death & Dismemberment Insurance pays a lump sum benefit to you or your beneficiaries:
- Life Insurance
- In the event of death.
- Accidental Death & Dismemberment (AD&D) Insurance
- In the event of serious injury or death as the result of an accident.
You will be automatically enrolled in Basic Life and Accidental Death and Dismemberment (AD&D) coverage, at no cost to you. You have the option to purchase voluntary, additional coverage for yourself, your spouse or your children.

* Voluntary Life and AD&D benefits and Employee and Spouse Basic Life benefit will reduce by 35% at age 65, and an additional 15% at age 70.
** Base earnings is the base compensation paid to the Employee excluding bonuses, incentives and overtime pay.
Who receives the benefit?
You can choose who should receive the benefit for your Life and AD&D Insurance by designating a beneficiary. It’s important to keep your beneficiary’s information up to date so that the benefit is paid according to your wishes.

How much will coverage cost me?
Your coverage cost is broken down to per $1,000 of coverage. For example:
Joe is 37 years old. Joe elects $10,000 voluntary coverage for himself.
- Breakdown of per $1,000 coverage in Joe’s elected coverage: 10,000 /1,000 = 10
- 10 x age banding ($0.078) = $0.78 per check cost to Joe
Joe’s wife is 34 and he elects $5,000 Voluntary Spouse Life coverage.
- Breakdown of per $1,000 coverage in the elected coverage: 5,000/1,000 = 5
- 5 x age banding ($0.055) = $0.275 per check cost to Joe
